What is your horse's current diet? Include details on hay, feeds and supplements! If weights are not known, use best description possible (ie, one 3QT scoop AM + PM; 7 small square bale flakes).

What is your horse's current living set up like? (Ie; out 24/7 in small herd with pasture, fed supplements once per day; stabled at night and fed AM and evening, etc).

What is your horse's workload like? How many hours per week are they exercised?

Does your horse have any health concerns? What are your goals from this consult?

Is your horse on any medications currently or within the last 6 months? If so, please describe below.

Preferred date & time for the nutrition consult/data collection session. Please note this is not guaranteed and that date & time will be confirmed with service booking confirmation. Virtual services can be via phone or video call based on your preference.

File upload - feel free to email these to honosnutrico@gmail.com instead if that is preferred! Pictures to include: horse body condition from side and rear, hay & pasture, feed scoops used if weight is not known, living situation (field or stall set up).

Upon receiving your form, I will be in touch to confirm your booking, as well as provide a price quote based on the services selected, number of horses and location (if booking an H-On Farm service, mileage fees may apply). Payment or deposit (based on service) will be required to confirm your booking. Feel free to reach out with any questions in the meanwhile!
